Home Base’s Community Based Services are offered in the following regions: Seacoast (Rochester office), Manchester, Nashua, Monadnock area (Keene office), and Concord (Pembroke office). Home Base Collaborative Family Counseling is certified to provide IHBT, ISO, and CHA (referred to as Parent Education / PE) throughout New Hampshire.

Parent Educator (PE)
Known to DCYF as CHA, our Parent Education Service is a psycho-educational parenting intervention including supported parenting time, i.e. supervised visitation, to assist parents in building skills, and removing barriers, in order to achieve REUNIFICATION. PE’s use Solution-Based Casework (SBC) to join with the families’ they service to create behavior change and enhance safety at both a family and individual level. SBC is nationally recognized and supported as an effective family-reunification tool.
Intensive Home-Based Treatment (IHBT)
Provides support, resources, advocacy, supported parenting time and mental health services. HBT is a short term service that strives to connect the family and child to community services, improve communication, and help the family increase natural supports.
Individual Service Option (ISO)
Provides all of the above HBT services as well as more intensive case management. ISO is the most intensive level of service and offers a combination of individual therapy, family therapy, school support, psychoeducation, case management, parenting education, and clinical mentoring. ISO also includes transportation support, respite, and crisis stabilization.
